# Field Observation Log

Use this record for live field evidence, a sanitized retrospective, or synthetic
practice. Preserve exact source passages and keep observation separate from
assertion, policy, and tested behavior. Synthetic reconstruction demonstrates
reasoning, not field observation or customer authority.

For self-directed practice, use the [fictional claims-denial case](claims-denial-practice-case.md)
and the [field conversation kit](field-conversation-kit.md). Label synthetic work as
practice evidence.

## Observation boundary

- Evidence mode: live field evidence / sanitized retrospective / synthetic practice:
- Workflow and case identity:
- Observation date and case date:
- Customer-safe scope:
- Observer:
- Process knower or operating owner:
- Why this case is representative—or the limitation that prevents that claim:

## Evidence-handling boundary

- Permission or consent basis for observation, reconstruction, recording, or
  transcription:
- Permitted source systems and passages:
- Prohibited sources, restricted fields, and redaction rules:
- People or roles allowed to review the retained record:
- Approved storage location, retention period, and deletion owner:
- Purpose limitation and prohibited reuse:
- How the operator or source owner may correct an inaccurate reconstruction:
- What must remain a reference or summary rather than copied content:

## Case path

| Step | Trigger or input | Actor or system | Decision | Evidence inspected | Action | System of record |
| --- | --- | --- | --- | --- | --- | --- |
| 1 | | | | | | |
| 2 | | | | | | |
| 3 | | | | | | |

## Exceptions and workarounds

- Exception observed:
- What changed in the path:
- Who decided:
- Source passage or system evidence:
- Frequency: observed once, measured, asserted, or unknown:

## Consequential claims

| Claim | Evidence basis | Exact source or passage | Applicability | Limitation | Status |
| --- | --- | --- | --- | --- | --- |
| | observed case / exact-build test / policy / assertion | | | | supported / contested / unknown |

## Role separation

- Sponsor:
- Process knower:
- Accountable operating owner:
- Scoped disposition authority:
- Basis for each role; do not infer one role from another:

## Current field view

- What is known:
- What is contested:
- What remains unknown:
- Constraint most likely to change the boundary:
- Next movement: validate the person / observe a case / resolve a cited conflict:
- Owner and date, when known:

## Independent review

- Reviewer:
- Can the reviewer distinguish evidence classes and limitations?
- Does the record avoid frequency, permission, or authority claims unsupported by the evidence?
- Review disposition:
